The Town Fork Creek Neighborhood Association is offering a limited Minor Home Repair Grant for qualifying homeowners. This program provides up to $5,000 per household to assist owner-occupants with minor repairs needed to maintain safe and healthy homes.
To be eligible, applicants must be members of the neighborhood association and meet the income guidelines listed below.
Income Eligibility
Income eligibility is dependent on total household income, starting at $36,000 for one person, and increasing by $7,500 for each additional household member.
For example, a family of 3 can have a maximum household income of $51,000 ($36,000+$7,500+$7,500).

Description of repairs you are requesting the minor home repair grant for

🏦 Household Income
List all income your household received in the last 12 months
For each income source, provide:
Name of household member who earned the income
Name of income source
Type of income (employment, social security, etc.)
Monthly income amount
🌐 Household Assets
List all savings and checking accounts (including IRAs, Keogh, 401(k), CDs, etc) of all household members.
For each account, provide:
Name of household member who holds the asset
Name of financial institution that holds the asset
Type of asset
Balance
Annual Income
Value of all stocks, bonds, trusts, pension contributions or other assets
